EMPLOYEE ASSISTANCE - BLOMQUIST HALE
An Employee Assistance Program (EAP) is a voluntary, work-based program that offers free and confidential assessments, short-term counseling, referrals and follow-up services to employees who have a personal and/or work-related problems.
This is a FREE service to eligible employees and their dependents of Nebo School District. Click on the logo above or call 801-262-9619 to contact Blomquist Hale direct and schedule an appointment.
You can see a therapist at one of their office locations anytime. Beginning September 2022, Blomquist will schedule patients 4 times a month at the Nebo Health and Wellness center. You can request that location if you choose when making your appointment.
You can also search the Blomquist Hale Youtube channel. There are many workshops on several different topics that they continually update on this page.
LIFE INSURANCE - HARTFORD
During the 2022-2023 Open Enrollment Period ONLY, Hartford has approved a guaranteed issue of $350,000 on employee voluntary life coverage and $50,000 on spouse voluntary life coverage with no medical questions or exam. If you elect amounts over the guaranteed amounts, you will need to send in the Evidence of Insurability Form to Hartford. The premiums on voluntary life are determined by age and other factors. Premiums will show when you select the amount of coverage you would like during the enrollment process.
*Life benefits reduce by 35% at age 65, 60% at age 70, and 75% at age 75.
Video Explaining Guaranteed Voluntary Coverage Amounts - This amount is only guaranteed during your open enrollment period. To receive this amount after the May 2022 Open Enrollment period will require an Evidence of Insurability form be sent in and Hartford will approve or reject based on medical questions and tests.
