Attention Parents/Guardians of Future Maple Grove Middle School Students:
Before beginning 7th grade, students must receive updated immunizations. Please see this Utah County Health Department flyer for more information. This immunization record may be turned in from now to the end of the school year at your student’s current elementary school or beginning July 1st you will be able to turn it in at Maple Grove Middle School.
