• Must provide a copy of students Birth Certificate.
• Students who do not live with a parent or court appointed legal guardian, or is coming from another country, must register with the District Office prior to registration. Contact Barbara Quackenbush at the Nebo School District Office.

Transcript

• Must provide transcript stating previous years' grades.
• UBSCT or other state competency scores.

Check Out Form from Previous School

• Must provide check out form stating student's check out grades.
• Behavior/Discipline Report from last High School.
• Attendance record from last High School.

Immunization Record

• Must include: 4 DPT, 3 Polio and 2 MMR.
• Out of country students must have proof of a current TB test from Utah County Health Department.

Address Verification

• You must provide one of the following: Current Utility Bill, Letter from Employer stating mailing address for payroll, Social Security, Government Assistance. Lease or rental agreement with landlord verification, current credit card statement.
• Drivers License or Personal checks are NOT ACCEPTABLE.
