Information for Register My Athlete

Mandatory Information for Participation In ANY School Sport This Year

Please be aware that Nebo School District requires that athletes who are trying out for any team sport have a current physical exam on file BEFORE trying out for the team. Exams are valid for one year. Parents will need to go to  www.registermyathlete.com to register their student and then upload the physical exam information as well as electronically sign other participation disclosures and information. No student will be allowed to try out for a sport without first completing the athletic registration at the Register MyAthlete.Com website. 

How To Register Your Athlete For Sports at Mapleton Jr. High

For 1st Time Registrations:

  • Go to www.registermyathlete.com

  • Click “Login” in top right corner

  • Select “Utah” from drop down menu

  • Select Mapleton Jr. High School

  • Select “Create and Account”

  • Fill out the necessary demographic information - this is the PARENTS INFORMATION, not the athlete yet!

  • Confirm you’ve read and agreed to the terms

  • Click on “Add a New Athlete” at the top of the page

  • Fill out your athlete’s information

  • Fill out Other Guardian information, if desired, and Emergency Contact Information

  • Provide Doctor’s Name and Phone Number

  • Provide your Insurance information or Opt to not provide it.

  • E-sign your insurance waiver, IF you opt out.  

  • *E-signatures must be EXACTLY the way you entered your name-middle initials, periods, etc…

  • Click “Register for a Sport” in top left corner

  • Select your student athlete’s sport(s)

  • Answer all the necessary questions concerning eligibility, rules and sportsmanship

  • E-sign at the bottom of the page

  • Click on each Document, read and click “I Agree” to confirm you have read and understand what each says

  • E-sign at the bottom of the page

Now you just need to upload your athlete’s physical exam form in order to try out for a sport. The physical exam is good for a full year once it is uploaded. If your student makes the athletic team they have tried out for you will need to pay the participation fee. Participation Fees can be submitted to the Finance Office or paid online through MySchoolFees.com.

General Registration Information for 2018-2019

Dear Students and Parents,

I hope you are having a wonderful summer vacation. Here at MJHS, the administration, counselors, secretaries and custodians are working hard to have everything prepared for the 2018-2019 school year. We are looking forward to the arrival of our students. The following information has been prepared to guide you through the ONLINE REGISTRATION PROCESS and to help you prepare to be ready to begin school on Tuesday, August 21st.

Online Registration (beginning August 1st)

To register your student(s) for the 2018-19 school year, go to the MJHS website and click the “Registration” link on the home page. (Or click this direct link: http://mjhs.nebo.edu/registration )  After completing the registration forms and paying the required fees your student’s class schedule and locker information will be accessible online by logging into your student’s regular Aspire/SIS account.  Please allow approx. 24-48 hours after both the paperwork and the payment are submitted for the information to appear. If you would like to pay your student’s school fees with cash or a check, you will need to first complete the online registration forms and then come into the school financial office to pay. Registration begins August 1st and must be completed by August 16th.  If you need assistance with the on-line registration process, computers will be available at the school.

Fee Waivers - August 6th through 10th

Fee Waiver qualifying information and income guidelines for the 2018-2019 school year is located on the school website by clicking this link: http://mjhs.nebo.edu/content/general-information-about-school-fees-and-f...

If you think you may qualify for a fee waiver, you must apply in person at the school from August 6th through August 10th, 8:00 a.m. - 4:00 p.m. Verification of income is required. Yearbooks and certain optional activities are not subject to fee waivers.

7th Grade Immunization Requirements

To attend the 7th grade in Utah a student must have proof of receiving the Meningococcal vaccine and the Tetanus/Diptheria/Pertussis (Tdap) booster. (The kindergarten Tdap does not count as the booster requirement for 7th grade.) This is in addition to other immunizations which should have already been completed and on file (i.e. MMR, DTaP, Polio, Hepatitis A, Hepatitis B, Varicella). You will not be able to attend 7th grade without the school having this information on file. Parents are asked to please bring in or mail in a copy of your student’s immunization document(s) to the school nurse, Diane Cooper before August 18th. You may also email an electronic copy of the immunization form to the school nurse at: diane.cooper@nebo.edu

7th Grade Day and MJHS Open House – Monday, August 20th   

All 7th graders are invited to attend 7th Grade Day on Monday, August 20th from 8:00-10:45 a.m. Students will have the opportunity to attend an orientation assembly, go to each of their classes, meet their teachers and practice opening their lockers. Buses will pick up bus students at their regular bus stops (If you need busing information please see the information under Busing). 

Later that same day from 1:00-4:00 p.m. there will be an Open House for students of all grades and their parents/guardians. Teachers will be in their rooms and available to answer any questions you may have.

Busing

The walking distance for a junior high student in Nebo School District is up to two miles. Students living farther than two miles are offered busing. If you are unsure whether your student qualifies for busing or you need to know where the bus stop is located, you can find this information at www.nebo.edu by clicking on Parents in the top menu bar and then clicking on Parent Home Page from the drop down menu. Locate the Bus Route Finder link in order to access your specific information. If you have further questions you can contact the transportation department directly at 801-465-6005.

School Times

The school day is from 8:00 a.m. to 2:40 p.m. - please note however that school will release early at 1:55 p.m. on Mondays in order to allow for teacher collaboration meetings. 

School Dress and Grooming

Parents - please be mindful of the Nebo School District policy when purchasing school clothes. Shorts and skirts must be mid-thigh length when the student is sitting. Shirts must cover the midriff and extend to the shoulder; they must not be low cut nor have inappropriate slogans. Thank you for your help with this policy. 

School Community Council

Mapleton Jr. High will be holding School Community Council elections for the 2018-19 school year August 20th through August 31st.  Parents or guardians of students attending MJHS are eligible to be elected.  If you are interested in becoming a candidate for one of the open seats on the council, contact Mr. Adams at 801-489-2892 or email mike.adams@nebo.edu by Friday August 17, 2018.  Voting will begin on Monday, August 20th.  For more information about School Community Council and the important role it plays to support MJHS visit http://www.schoollandtrust.org

We appreciate the dedicated service of our community council. This past year the council worked hard to equitably allocate school LAND Trust money in the support of student learning. This money helps improve academic achievement by providing student intervention programs, student technology, reducing class size, providing professional development and helping us support our school improvement plan. School Lunch

Breakfast and lunch is served daily at the following prices:

▪ Breakfast: $1.50 (reduced - $.30)    ▪ Lunch:  $2.50 (reduced - $.40) 

To pay for lunches online: visit the Nebo District - Parent Home Page https://www.nebo.edu/parents and click “Pay for meals online”. You will need to create a PayPAMS account if you have not already done so.

Free/Reduced Lunch: applications must be completed online. To apply for free or reduced lunch visit the Nebo District - Parent Home Page https://www.nebo.edu/parents and click on “Free & Reduced Lunch Information”.  Please do this as soon as possible so your student can receive free/reduced lunch on the first day.

Summer Food Bank Lunches

Utah Food Bank - summer meal program in Utah County will be offering lunches this summer.  All youth up to age 18 may receive a free healthy lunch. The program begins June 11th and will continue through August 17th, Monday through Friday, 1:45 p.m. to 3:00 p.m.  Lunches will be served at the Springville Splash Pad, 50 South Main Street. If You Are Attending Lagoon

Wednesday, May 23rd is Lagoon day - Buses will leave promptly at 8:00 a.m. Please arrive at school by 7:40 a.m. in order to load the buses.  Because we have to pay the bus company for your student’s seat whether they go or not, refunds will not be available to those that miss the bus. In order to accurately account for each student, they will need to ride the same bus up and back.  On the bus ride up students will be informed that buses will leave Lagoon promptly at 3:30 p.m. We estimate that we will be home between 4:30 and 5:00 p.m. Some parents have asked about picking up their own student at a prearranged time. If you are planning on picking your student up at Lagoon rather than having them ride the bus home please provide a parent/guardian note, signed and dated, giving permission to your student to be released.  Students must give this note to the teacher as they board the bus here at MJHS so that we are aware of the situation. Because of student safety concerns, only students that provide a written note will be permitted to be released and not ride the bus home. School will be held during normal hours for those students who choose not to go to Lagoon or are not eligible to attend.

Crystal Apple Recipient - Lori Marett

This year’s Crystal Apple Award recipient is Lori Marett. Lori is a dedicated educator who cares deeply about student success. For many years she worked as an English teacher, ESL instructor and built the High Honors Program.  More recently, she has transformed the library into a high-interest, student-centered environment where all feel welcome. Lori spends countless hours helping students and teachers, planning programs to encourage reading, such as Battle of the Books and March Madness.  In addition, she researches the best books for the library. It’s not unusual to see Lori talking with students about their favorite books. She encourages students to write what books they want to read on her book recommendations list because she values their opinions. The library is the heart of the school, and Lori is a vital part of our school’s success.

Not only is Lori a phenomenal librarian, she is also a valued member of the faculty and a dear friend.  Congratulations Lori!

MJHS has Three Top Winners at District Poetry Slam

Last night, eleven students from Mapleton Junior competed in the Nebo District Poetry Slam. This is the first time that Mapleton Junior has participated. The students who competed were Heather Ells, Willow Schouten, Kaitlyn Cragun, Jasmin Young, Adam Taylor, Ammon Brown, Lila Blaser, Anna Ballard, Chelby Carter, Tate Hickman, and Samuel Smith. Samuel Smith received an honorable mention for his performance of Neil Gaiman's "Crazy Hair," Heather Ells received fourth place for her poem "Life Through Lenses," and Adam Taylor received second place for his poem "Hiraeth." Great work Mustangs!

Spirit of the Mustang Recipient - Leslie Lines

When you think about teachers that greatly contribute to the family atmosphere of Mapleton Junior High,
Leslie Lines is one of the first teachers that comes to your mind. Leslie is always willing to go the extra
mile for her school. She teaches huge classes of energetic kids, coaches multiple sports, and volunteers
as much as possible for extra responsibilities. Leslie does all of this with a smile on her face. Not only
does she do a fantastic job at the school, she is a loving and responsible mother. I admire the amount of
time and attention that she gives her son Aiden and how good she is with him. Leslie is very fun as well.
She is very good at entertaining and teaching at the same time. She is a great example of a Mustang!

MJHS Battle of the Books District Winners!

On April 30, 2018 Mapleton Jr. High School and Diamond Fork Jr. High School met in the library at Diamond Fork Jr. High for a district Battle of the Books for Jr. High. We had a lot of fun. The team "ShardBearers" from Mapleton Jr. High School won. After winning the MJHS Battle of the Book competition, the Shard Bearers (left to right are Joslynn Wheeler, Kyler Korth, Casey Harris, Ben Johnson, and Olivia Memmott).  MJHS had 63 students on 13 teams compete in their Battle of the Books competition.  The first and second place teams competed against two teams from Diamond Fork for the district championship.  The list for next year's Battle of the Books is available in the library.
